Santa Fe – Presbyterian Medical Services (PMS) is proud to announce that PMS – Western New Mexico Medical Group – Grants is one of five PMS health centers to achieve the status of Patient-Centered Medical Home (PCMH) Recognized Practice from the National Committee for Quality Assurance (NCQA*) PCMH 2011 Recognition Program. NCQA identifies and recognizes practices that deliver superior care using evidence-based, patient-centered processes that focus on highly coordinated care and long-term participative relationships with the patients, families and communities they serve.

WNMMG – Grants received PCMH Recognition (Level-1) certification for their accomplishments across a range of standards involving all aspects of care delivery including clinical operations, utilization of the electronic health record, care coordination and outreach for their patients with chronic care needs, quality improvement and population health activities. Lita Bailly, Nurse Practitioner; Marla Mitchell, Administrator; and Karna Patel, M.D., Medical Director celebrate national Recognition as a Patient-Centered Medical Home.

WNMMG – Grants joins 3,000 practices nationwide to achieve any level of PCMH Recognition. This accomplishment marks a major milestone toward the PMS Corporate Strategic Initiative of achieving PCMH Recognition (at least Level 1) across all eligible PMS clinical sites in 2012. A special luncheon was held at the center on August 22,  2012, to celebrate this outstanding achievement. It was an opportunity for PMS to applaud the hard work of the site’s PCMH team, their providers, and staff in this confirmation that the PMS approach to patient-centered primary care is recognized as a model and standard of excellence.
